Employee SMS Voicemail Notification — Consent Form
Snapp Dental offers employees the option to receive a text message on their mobile phone when a new voicemail arrives in their office phone extension or mailbox. Enrollment is voluntary and is handled on paper: an employee completes and signs this form in person at our office at 827 128th Street SW, Suite D, Everett, WA 98204, during onboarding or at any time afterward, and the office manager then enters the mobile number into our phone system.What you will receive
- Message type: voicemail notifications only — a text telling you that a new voicemail is waiting, and the caller ID and time of the call.
- Frequency: varies; one message per voicemail received. No marketing or promotional messages are sent to this list.
- Cost: Snapp Dental does not charge for this service. Standard message and data rates from your carrier may apply.
- Stopping: reply STOP to any message to be removed immediately, or ask the office manager to remove your number. Reply HELP for help, or call (425) 353-0110.
